Finding a contracted provider

Step 1

1. Visit dmba.com.
2. Navigate to Find a Provider and under Mental/Behavioral Health Providers select Utah and SE Idaho (DMBA).

Step 2

1. Select the appropriate State, County, and Medical Plan from the drop-down menus.
2. Select Gender and Language preferences if needed.
3. If medication management is needed, select yes under Prescriptive Privileges.
   
   • By selecting this box, your results will be narrowed to advanced practice registered nurses (APRNs), physician assistants (PAs), medical doctors (MDs), and doctors of osteopathic medicine (DOs).
4. Check the box next to the desired specialties, up to two (optional).
5. Click on Find Providers. A list of providers displays alphabetically, based on the search criteria. Select a provider’s name to view details about that provider.

Step 3

1. A dialogue box displays specific details about the provider’s practice locations, education, board certifications, and contracted DMBA plans.
2. A link to a map of the provider’s location(s) is available on this page.

Providers are only contracted at listed locations.

Step 1

1. Visit dmba.com.
2. Navigate to Find a Provider and under Mental/Behavioral Health Providers select Hawaii (MDX). (Alternatively, you may select Hawaii (DMBA) and follow the steps outlined above.)

Step 2

1. On the MDX Hawai‘i site, select Commercial Health Plans.
2. Select the desired island.
3. Select the Specialty.
4. If you have preferences for provider names, gender, or ZIP code, enter those search terms.
5. Click Search.

Step 3

1. Choose a provider from the displayed list.

Step 1

1. Visit dmba.com.
2. Navigate to Find a Provider and under Mental/Behavioral Health Providers select United Healthcare (UHC).

Step 2

1. On the United Healthcare Behavioral Health Care site, select your desired location by clicking Change Location > if the default location is incorrect.
2. Select from the Frequent Services or Find Care By Category.

Step 3

1. Refine the results using the filter on the left.
2. Choose a provider from the displayed list.
